Grace at Home provides personalized, one-on-one, non-medical in-home care with customized care plans and schedules as unique as each client we serve.

 

You and your loved-one will take the lead in helping us create a unique care plan and appropriate schedule based on your individual needs and preferences.  

 

 

You’ll work with our professional caregiving staff, and together we will come to understand exactly what you need and expect.  

 

The process isn’t complicated, but it is thorough, and foundational to the best in-home care experience. With your help, we will create a customized care plan that will help keep your loved-one safe, encouraged, and strong.


 

Your care team will always encourage your loved-one to be as active and independent as possible, within his/her own capabilities.

 

We understand that each client we serve has a unique personality with wonderful gifts and values that must be honored. 

 

Care plans and schedules are unique to each client and can quickly be updated or changed to meet your loved-one’s current needs or changing situation.


You may need only a one-time visit for respite, care just a few hours a day several days a week or maybe around the clock care, 7 days a week.
